Test Playback Failed- URL mismatch

I'm constantly getting URL mismatch error among other ones that I get. Haven't had a problem creating monitors in the beginning but now I'm always getting some error so I haven't been able to create a monitor in a while. I was hoping someone might know what the problem might be and how to fix it.
